On the heart line an island, regardless of location, in my perspective of hands is confusion. It's a line going two directions so there isn't a clear path.
On the heart line it's about confusion regarding feelings or emotions. Under Apollo (ring finger) it's specifically confusion in your heart/feelings about your artistic expression at one level. Are you confused about a direction or that you have artistic desires?
It can also be about relationships. There are some markings related to the Apollo energy about betrayal (an X off the heart line under Apollo) so maybe your challenge is confusion about a relationship regarding possibly not fitting in or feeling connected.
